No, there isn’t paint damage when fixing normal dents, dings, creases, and hail damage. The only time the paint could be compromised would be if the dent is severely sharp or the metal has been extremely stretched. In the case of severe damage the customer will be warned about this before repair.
The paintless dent removal process uses tools that push on the backside of the metal. Basically the dent gets pushed out until it is back to perfect. If the push goes too far, the push is tapped back down to flat. Eventually, between pushing the dent up and tapping down the high spots, you have the finished product.
